An adolescent girl fell three stories down an elevator shaft in Soho Friday afternoon, but is said to be OK.
Authorities said the girl was on the first floor of the Crosby Street building and somehow fell about 30 to 35 feet down the shaft to the sub-basement.
She was conscious when firefighters rescued her, officials said. She had a broken arm.
According to Department of Buildings records, the elevator failed its annual load test in June 2013.
The building was also cited in December for failure to maintain the elevator, which resulted in violations like oil spills, leaks and expired device tags.
The Department of Buildings has ordered the elevator out of service as it investigates.
