Transit officials say an investigation into a subway derailment this year that injured 30 riders and two employees was caused by multiple track defects.
An MTA report released Friday finds that inspectors didn't document and investigate a broken plate, fasteners and a deteriorated tie on the Queens subway line.
The report says a combination of defects caused the May 2 derailment of an eight-car F train that caused $2 million in damage.
About 1,000 commuters were on board when the train went off the rails.
The report recommends disciplinary action against three maintenance supervisors and a track inspector.
TWU Local 100 President John Samuelsen says in a statement the MTA report's recommendation for disciplinary action is scapegoating. He says the rail was defective.
